Oligio RF vs HIFU: which is right for you?

Monopolar radiofrequency and HIFU both tighten and rejuvenate — but they work differently and suit different patients. A clinical comparison for UK practitioners.

How Oligio Works

Oligio is a monopolar RF device developed by Wontech in South Korea, with a body of published clinical research supporting its mechanism and outcomes.

The electromagnetic field generated by radiofrequency stimulation alters the polarity of particles in the skin tissue approximately 6 million times per second. These rapid movements generate high-frequency electrical currents. The natural resistance of the skin tissue converts these currents into volumetric heating, causing controlled damage to collagen fibrils — triggering immediate contraction, followed by the stimulation of new collagen synthesis.

To prevent surface damage, contact cooling is implemented using a cryogen spray on the inner surface of the treatment tip before, during, and after each energy pulse.

Clinical outcomes documented in peer-reviewed literature include improvements in skin firmness, elasticity, and texture — particularly in the mid and lower face — with minimal downtime and strong patient satisfaction rates.

How HIFU Works

HIFU delivers focused ultrasound energy to deeper tissue layers, including the SMAS — the superficial musculoaponeurotic system, the structural foundation of the face.

HIFU is a non-invasive, non-ablative rejuvenation technique that uses ultrasound waves to penetrate the epidermis, inducing vibration of cellular molecules and leading to thermal injury at the SMAS layer. This effectively thickens and shortens collagen fibers, increases tissue tension through the elastic properties of collagen, and tightens the skin — with minimal undesirable epidermal injury irrespective of Fitzpatrick skin type.

The thermal injury triggers the body’s natural wound-healing response. Over the following weeks and months, fibroblasts are activated to repair the coagulation points — resulting in neocollagenesis and elastin remodelling. This new collagen matrix is ​​stronger and tighter, providing sustained structural support. The full results of a HIFU treatment typically unfold over two to three months.

Can They Be Combined?

Increasingly, yes — and this is where Korean combination protocol thinking becomes particularly relevant.

A 2025 prospective trial published in the Journal of Cosmetic Dermatology found that both RF and focused ultrasound produced statistically significant improvements in facial wrinkle scores within three months — with the ultrasound group showing superior mid-face and lower-face efficacy at one month, and both groups demonstrating strong intragroup improvement over the study period.

Since Oligio targets the dermis and HIFU targets the SMAS, the two technologies address truly different tissue depths. A well-designed combination protocol can therefore produce more comprehensive rejuvenation than either device alone — provided the treatment plan is individualized to the patient’s anatomy, skin condition, and aesthetic goals.
